General Information |
With this form, you can pre-register for ICMS courses or seminars, and/or request certain types of information.
To pre-register for classes, make your choices from the pull-down lists. Submitting the form will pre-register you for the selected courses, save your spot at those events, and secure your early registration discount if applicable. ICMS staff will send you a confirmation via regular mail, with a total tuition due. Upon receipt of that confirmation, you must submit the appropriate fees to the ICMS or risk losing your spot in the course or seminar.
To request information, such as brochures for Affiliate Conferences, or a Certificate Program brochure, make the appropriate selections on this form and submit it. (Note: You do not have to register for classes to use the form to request information). ICMS staff will send you the information via regular mail.
